Change in Status (CIS) Form Procedure
If you need to amend a hire form for a position that has already been approved by our office, please send [email protected] a Change in Status (CIS) form.
Changes that require a CIS Form include:
- Change in pay rate
- Change in hire date or termination date
- Change in Banner Index or account number
- Change in the number of hours working per week (FTE)
If the position is still in our approval queue, you can cancel the hire request and submit a new request with the correct information. You can check a request's approval status in the Faculty Hire Administration interface via "View Submitted/Approved Faculty Hire Forms".
